Complete Guide to Understanding and Treating Hemorrhoids

What are Piles (Hemorrhoids)?

Piles (also known as Hemorrhoids) are the swelling of blood vessels near the anal opening. They are described as a "common condition in both men and women," often found in adults between the ages of 20 and 50 years. The condition involves swollen and enlarged veins that form inside and outside of your anus and rectum.

Internal Hemorrhoids

Located inside the rectum, usually painless but may bleed.

External Hemorrhoids

Located under the skin around the anus, can be painful if thrombosed.

Causes of Piles

Straining During Bowel Movements

Chronic constipation or diarrhea can cause excessive straining, leading to swollen veins in the rectal area.

Sedentary Lifestyle

Lack of physical activity and prolonged sitting can increase pressure on the rectal veins.

Pregnancy

Increased pressure from the growing uterus and hormonal changes can contribute to hemorrhoid development.

Obesity

Excess weight puts additional pressure on the pelvic floor and rectal area.

Chronic Cough

Persistent coughing increases intra-abdominal pressure, which can affect rectal veins.

Symptoms of Piles

Common Symptoms

  • Painless bleeding during bowel movements
  • Itching and irritation around the anus
  • Pain and discomfort during bowel movements
  • Swelling and lumps around the anus
  • Mucus discharge

When to See a Doctor

  • Severe pain or bleeding
  • Symptoms persist for more than a week
  • Blood in stool or black stools
  • Fever or chills

Grades of Piles

Grade I: Mild

Internal hemorrhoids that bleed but do not prolapse. Usually painless and may not require treatment beyond lifestyle changes.

Grade II: Moderate

Hemorrhoids that prolapse during straining but return spontaneously. May require minimal intervention like rubber band ligation.

Grade III: Severe

Hemorrhoids that prolapse during straining and require manual reduction. Often painful and may need surgical treatment.

Grade IV: Very Severe

Hemorrhoids that are permanently prolapsed and cannot be reduced. Require immediate surgical intervention.

Investigations for Diagnosis

The diagnosis of Piles (Hemorrhoids) is primarily based on a physician's physical examination and assessment of symptoms. To confirm the diagnosis and determine the exact grade, the following investigations are commonly used:

Diagnostic Methods

  • Proctoscopy: This is the most common and quick diagnostic test. It uses a short, hollow instrument to view the lower part of the anal canal and the rectum. This helps in confirming the presence and grade of Internal Piles.
  • Anoscopy: This examination is similar to Proctoscopy. It provides a more detailed inspection of the innermost lining of the anal canal.
  • Colonoscopy: This is often prescribed when the doctor suspects the bleeding might be caused by an issue in the large intestine (colon) other than Piles, or for a full screen, especially in older patients. It examines the entire colon and the rectum.

What to Expect

  • • Most procedures are painless
  • • Minimal preparation required
  • • Results available immediately
  • • No anesthesia usually needed

Complications of Piles

Thrombosis

Blood clots can form in external hemorrhoids, causing severe pain and swelling that may require surgical intervention.

Strangulation

Prolapsed hemorrhoids can become trapped outside the anus, cutting off blood supply and causing tissue death.

Infection

Open wounds or ulcers can become infected, leading to abscesses or cellulitis that may spread systemically.

Anemia

Chronic bleeding from hemorrhoids can lead to iron deficiency anemia, causing fatigue and weakness.

Anal Fissure

Chronic hemorrhoids can lead to the development of anal fissures due to increased pressure and trauma.

Myths and Facts About Piles

Myth: Piles only affect older people

Fact: While more common in older adults, piles can affect people of any age, including young adults and even children.

Myth: Surgery is always required for piles

Fact: Most piles can be managed with lifestyle changes, medications, and minimally invasive procedures. Surgery is typically reserved for severe cases.

Myth: Piles are contagious

Fact: Piles are not contagious and cannot be transmitted from person to person. They are caused by lifestyle and physiological factors.

Myth: Creams and ointments cure piles permanently

Fact: Topical treatments provide symptom relief but don't cure piles. They must be combined with lifestyle changes for long-term management.

Treatments of Piles

The treatment approach for Piles (Hemorrhoids) is directly related to the grade or severity of the condition.

Early Grade

Grade 1: Mild pain, mild bleeding, itching, and heaviness. No Prolapse (piles remain inside the anal opening).

Can be treated with medicines & conservative measures (like diet changes).

Note: Other non-allopathic methods should be discussed with a specialist.

Late Grade

Grade 2: Symptoms of Grade 1 with Prolapse that goes back inside on its own after motion.

Can be cured with Laser treatment.

Grade 3: Prolapse requires manual finger reduction after motion (must be manually pushed back in).

Can be cured with Laser and Stapler Surgery.

Grade 4: Prolapse is Irreducible (cannot be pushed back) and symptoms worsen.

Can be cured with Laser and Stapler Surgery.

Overview of Modern Techniques

Two advanced surgical techniques are highlighted as being preferred for treating Grade 2, Grade 3, and Grade 4 Piles:

LASER: The European Way for Cure
